Supports Coordination Services

Support Coordinators are trained professionals who help individuals navigate the complex landscape of services available to them. They assist consumers with accessing and managing the services and supports they need to lead fulfilling lives through the development of an Individual Plan of Service (IPOS).

Support Coordinators help with

• Resource Identification: Locating and recommending services that align with the individual’s need, including home-health and community-based services.
• Empowerment: Encouraging individuals to make informed choices about their care and support options, promoting autonomy and self-sufficiency.
• Coordination: Ensuring seamless integration of services by facilitating communication among various service providers and stakeholders.
• Assessment: Evaluating the individual’s specific needs to determine appropriate services and support.
• Monitoring: Regularly reviewing and adjusting services to adapt to the individual’s evolving needs and goals.

Program Requirements:

• Person must have an intellectual or developmental disability.
• Person must be 18 years old or older.
• Person must be a resident of Detroit or Wayne County.
• Must be Medicade eligible and enrolled.
